Our Mental Health Social Care Team (South) is a specialist team that plays a pivotal role in commissioning and reviewing a wide variety of care packages for citizens who are in contact with secondary mental health services. Our goal is to enhance the quality of life for those we serve by fostering independence and ensuring comprehensive support.
We are seeking a passionate and dedicated Qualified Social Worker (QSW) to join our dynamic Mental Health Social Care Team. As a QSW, you will:
Commission and Review Care Packages: Undertake initial assessments and submit care packages for approval, ensuring they meet the individual needs of our citizens.
Report Writing: Regularly prepare detailed reports for Mental Health Review Tribunals.
Mental Health Act Assessments: Conduct assessments as an Approved Mental Health Professional (AMHP).
Capacity Assessments: Formally assess the capacity of citizens to make key decisions about various aspects of their life.
Specialist Service Provision: Provide targeted support for citizens dealing with severe drug and alcohol-related problems.
Key Responsibilities
Conduct thorough assessments of individuals in contact with secondary mental health services.
Develop, commission, and review personalized care plans aimed at promoting independence.
Prepare and present comprehensive reports for Mental Health Review Tribunals.
Carry out Mental Health Act Assessments (AMHP) to ensure the rights and needs of individuals are met.
Assess and document the capacity of individuals to make informed decisions about their care and wellbeing.
Provide specialized support and intervention for individuals with severe substance misuse issues.
Qualifications and Experience
Qualified Social Worker (QSW) with a valid registration.
Experience working within mental health services.
Approved Mental Health Professional (AMHP) qualification is desirable.
Strong assessment, analytical, and report writing skills.
Excellent understanding of mental health legislation and best practices.
Ability to work collaboratively within a multi-disciplinary team.
