Job Description:
We are seeking a dedicated and experienced Qualified Social Worker to join our fast-paced Multi-Disciplinary Team at Bristol Royal Infirmary. This locum role offers flexible working hours with the expectation of being present in the office all five days of the week.
Key Responsibilities:
Work closely with health colleagues and community agencies to ensure a seamless service for our users.
Apply a comprehensive working knowledge of the different pathways within hospitals.
Conduct Care Act Assessments to evaluate and address the needs of service users.
Perform Mental Capacity Assessments and facilitate Best Interest Meetings.
Carry out reviews as necessary to ensure the continuous appropriateness of care plans.
Collaborate within a Multi-Disciplinary Team (MDT) to ensure safe and efficient discharge plans for service users, tailored to meet their individual needs.
Qualifications & Experience:
Must be a Qualified Social Worker with relevant registration.
Proven experience in a similar setting, preferably within a hospital environment.
Strong understanding of hospital pathways and discharge processes.
Excellent assessment and review skills, particularly concerning the Care Act and Mental Capacity Act.
Ability to work effectively within a Multi-Disciplinary Team and engage with various health and community professionals.
