We are seeking Senior Social Workers to join our team based at a busy Acute Hospital in Boston, Lincolnshire. Our focus is on completing proportionate assessments to support timely discharge from hospital, utilizing Home First principles and strengths-based practice to facilitate discharge to the individuals own home whenever possible. This role requires collaboration with families, carers, multi-disciplinary teams, and stakeholders from the Health and Voluntary sectors.
Role Overview:
As a Senior Social Worker, you will play a crucial role in supporting individuals through the discharge process from hospital to home. This includes conducting assessments, working collaboratively with families and carers, and liaising with multi-disciplinary teams to ensure holistic support. The role involves supporting junior team members and ensuring compliance with relevant legislation and best practice standards.
Key Responsibilities:
Conduct proportionate assessments to facilitate safe and timely discharge from hospital.
Implement Home First principles and strengths-based practice to promote independent living.
Collaborate effectively with families, carers, and multi-disciplinary teams to develop and implement discharge plans.
Provide support and mentorship to junior members of the team, ensuring high-quality service delivery.
Maintain knowledge and application of the Care Act, Mental Capacity Act, Continuing Healthcare, and Safeguarding practice in daily operations.
Requirements:
Qualified Social Worker with significant experience in a hospital or acute care setting.
Thorough understanding of the Care Act, Mental Capacity Act, Continuing Healthcare, and Safeguarding practice.
Ability to work collaboratively with Health and Voluntary sectors to support discharge planning.
Strong communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders.
Flexibility to work as part of a rota, including weekends as part of the 7-day service.
