Job Description
Are you a dedicated and passionate Qualified Social Worker looking for an exciting opportunity to make a difference in the lives of young adults? We are currently seeking a dynamic individual to join our Young Adults Transitions team in Bristol. This role offers a competitive rate of £32 per hour and the chance to work in a supportive and collaborative environment.
Key Responsibilities
Assessment and Planning: Conduct comprehensive assessments of young adults transitioning from childrens services to adult services. Develop, implement, and review care plans tailored to their individual needs.
Support and Advocacy: Provide ongoing support and advocacy to young adults and their families, ensuring they have access to the necessary resources and services during their transition.
Collaboration: Work closely with multi-disciplinary teams, including healthcare professionals, education providers, and other agencies to deliver coordinated care and support.
Safeguarding: Ensure the safety and well-being of young adults by identifying and addressing any safeguarding concerns in accordance with relevant policies and procedures.
Record Keeping: Maintain accurate and up-to-date records of all interactions, assessments, and care plans in line with legal and organizational requirements.
Requirements
Qualifications: A degree in Social Work and current registration with Social Work England.
Experience: Proven experience in working with young adults, particularly those transitioning from childrens to adult services.
Skills: Strong assessment and care planning skills, excellent communication and interpersonal skills, and the ability to work effectively within a multi-disciplinary team.
Knowledge: In-depth knowledge of relevant legislation, policies, and procedures related to young adults and transitional care.
