Lincolnshire County Councils Adult Frailty & Long Term Conditions (AFLTC) team is committed to providing high-quality care and support to adults and older people over the age of 65. Role Overview:
We are seeking an experienced and knowledgeable Qualified Social Worker to join our AFLTC team. The successful candidate will conduct Adult Needs Assessments within the community, working with adults and older people over the age of 65. This role requires a strong understanding of Mental Capacity Assessments, Best Interest decisions, and a strengths-based approach. The ability to support and guide colleagues and demonstrate person-centred practices is essential. This position involves remote and occasional office-based work, and driving is required due to the extensive area covered by the West Adult Frailty and Long Term Conditions Service.
Key Responsibilities:
Conduct Adult Needs Assessments in line with the Care Act 2014 for adults and older people over the age of 65.
Perform Mental Capacity Assessments and make Best Interest decisions.
Work in a strengths-based manner to promote independence and well-being.
Support and guide colleagues, both qualified and unqualified, in delivering high-quality care.
Demonstrate and implement person-centred practices in all aspects of work.
Maintain accurate and up-to-date records in compliance with legal and organizational standards.
Participate in case conferences, reviews, and team meetings.
Travel within the region as required, working both remotely and from the office on occasion.
Qualifications and Experience:
Degree in Social Work and registration with Social Work England.
Proven experience in adult social care, particularly with adults and older people over the age of 65.
Excellent understanding and experience in conducting Mental Capacity Assessments and making Best Interest decisions.
Strong assessment, planning, and organizational skills.
Excellent communication and interpersonal abilities.
Ability to work effectively both independently and as part of a multidisciplinary team.
Commitment to ongoing professional development and reflective practice.
Full UK driving license and access to a vehicle for work purposes.
