Pay Rate: £41 per hour
About the Role:
An exciting opportunity has arisen for a Social Work role that will focus on NHS Continuing Healthcare (CHC). The successful candidate will:
- Have recent experience and a thorough working knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022, Revised) and The Care Act (2014).
- Possess excellent assessment skills and the ability to identify care needs within the responsibility of the Local Authority.
- Work within a Multidisciplinary Team (MDT) approach, undertaking Social Care Assessments and Decision Support Tools for individuals identified for CHC Eligibility consideration.
Key Responsibilities:
- Conduct Social Care Assessments.
- Undertake Decision Support Tools for CHC Eligibility.
- Work predominantly from home, with visits to individuals throughout Lancashire.
Requirements:
- Proven experience with Liquid Logic, as this project does not provide training.
- Strong knowledge of NHS Continuing Healthcare and The Care Act (2014).
Details:
- Duration: Initially 8 weeks, with the possibility of extension.
- Working Environment: Primarily remote, with necessary visits within Lancashire.
