Warrington Borough Council is seeking a dedicated and compassionate Social Worker to join our Community Discharge to Assess (D2A) Team.
Role: Adults Social Worker
- Pay Rate: £30 per hour
- Location: Warrington
- Team: Community Discharge to Assess Team
Key Responsibilities:
- Conduct comprehensive Care Act assessments for individuals discharged from the hospital into temporary 24-hour care placements
- Assess and evaluate the physical, mental, and emotional needs of individuals to determine the appropriate level of care and support
- Collaborate with individuals, their families, and healthcare professionals to gather relevant information and develop comprehensive care plans
- Provide advice, guidance, and emotional support to individuals and their families to help them understand their options and make informed decisions
- Liaise with community resources, such as healthcare providers and voluntary organizations, to ensure seamless provision of services
- Advocate for individuals' rights and entitlements, ensuring timely and appropriate support services
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes
- Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services
Requirements:
- Bachelor's or Master's degree in Social Work
- Valid social work registration
- Valid driver's license and own car with business insurance
- Proven experience in conducting assessments and developing care plans
- Essential experience in Discharge to Assess (D2A)
- Sound knowledge and application of the Care Act, Mental Capacity Act, and Best Interest Processes
- Knowledge and experience of Continuing Health Care processes and participating in DST's (desirable but not essential)
- Excellent communication skills, both verbal and written
- Effective organizational and time management skills
- Strong interpersonal skills and the ability to build rapport with individuals, families, and multi-disciplinary teams
- Proficient computer skills, including experience with electronic record-keeping systems
- Ability to provide 3 years of references and verify gaps in employment history
