Southend City Council is expanding its Prevention Service and is seeking dedicated Social Workers for full-time permanent positions in the Adult Social Care Community Team. With a pay rate of up to £42,403 per annum, this is a fantastic opportunity to be part of a dynamic and innovative team.
Role Overview:
As a Social Worker in the Community Team, you will use a strength-based approach to support individuals returning home after hospital stays and enable them to live safely in the community. Your role will involve maximizing their abilities to care for themselves and providing essential support to carers, ensuring they can continue in their caring role as long as possible. You will be part of a team that prioritizes high-quality support, creativity, and innovation.
Key Responsibilities:
* Conduct strength-based assessments to support individuals returning home after hospital stays.
* Develop and implement care plans that enable individuals to live safely and independently in the community.
* Provide support and resources to carers, enhancing their ability to continue their caring roles.
* Collaborate with multidisciplinary teams to deliver comprehensive and effective support services.
* Participate in service development and contribute to innovative practices within the Prevention Service.
Key Requirements:
* Social Work qualification and registration with Social Work England.
* Experience in adult social care, particularly in community settings.
* Strong understanding of strength-based approaches and person-centered care.
* Excellent communication and interpersonal skills.
* Commitment to delivering high-quality support and fostering innovation within the team.
