Eden Brown Synergy is delighted to present a fantastic opportunity for a Social Worker to join the Ageing Well West Team at Southend-on-Sea Borough Council. This is a part-time role, working 30 hours per week, offering a chance to make a significant impact on the lives of citizens across Southend-on-Sea, including out-of-borough cases.
* Salary Range: £36,648 - £42,403 per annum
About the Role:
* Supporting Citizens: Provide timely and appropriate support to individuals, maximizing their potential for independence by understanding their strengths, assets, and personal goals.
* Assessment & Care Planning: Conduct assessments and collaborate with individuals to create personalized care plans that utilize community assets, informal networks, and universal services to reduce dependency.
* Innovative Practice: Engage in visionary practices with health and social care colleagues and other partners to create a better Southend.
* Community Engagement: Participate in reducing isolation, loneliness, and marginalization among adults and families by engaging with the community and promoting co-production.
Key Responsibilities:
* Act as a catalyst for change, embracing innovation and ensuring that solutions are based on recognized best practices.
* Collaborate closely with statutory partners, health, and social care professionals to ensure effective coordination and resolution of issues.
* Utilize a strengths and asset-based approach to practice in assessments, care plans, and the team culture.
* Take professional responsibility for safeguarding enquiries, Mental Capacity Act assessments, and Deprivation of Liberty Safeguards.
What You Need to Succeed:
* Experience: Minimum of 2 years post-qualification experience in a relevant setting, with a solid understanding of the Care Act (2014) and Mental Capacity Act (2005).
* Registration: Must be registered with Social Work England.
* Enhanced DBS Check: This role requires an Enhanced Adults Barred List check from the Disclosure & Barring Service.
* Skills: Strong analytical skills, the ability to develop effective relationships with stakeholders, and a commitment to continuous professional development.
