About the Role
Wiltshire Council is looking to recruit a dedicated and hardworking Qualified Social Worker to join our Hospital Social Work Team at Great Western Hospital. This is an exciting opportunity for individuals with a strong background in adult social care who are looking to make a significant impact on the lives of patients transitioning from hospital to community settings.
Key Responsibilities
Hospital Discharge: Facilitate and manage the timely discharge of patients from the hospital, ensuring all care needs are identified and met.
Assessments: Conduct comprehensive assessments of patients' needs and develop personalized care plans.
Care Coordination: Work closely with healthcare professionals, patients, and their families to coordinate care and support services.
Legislative Compliance: Ensure compliance with all relevant legislation and policies, including the Care Act 2014, Safeguarding, and the Mental Capacity Act 2005.
Advocacy: Advocate for patients' rights and best interests, ensuring they receive the care and support necessary to promote independence and well-being.
Multi-disciplinary Teamwork: Collaborate effectively with a multi-disciplinary team within the hospital setting to provide holistic and person-centered care.
Qualifications and Experience
Education: Degree in Social Work, Dip SW, CQSW, or equivalent.
Registration: Current registration with Social Work England.
Experience: Demonstrated experience in adult social work, particularly within a hospital or healthcare setting.
Knowledge: Strong understanding of current legislation and policies related to adult social care, including the Care Act 2014 and the Mental Capacity Act 2005.
Skills: Excellent communication, assessment, and care planning skills. Ability to work independently and as part of a team.
