Job Description:
We are seeking a Qualified Social Worker (QSW) to join our Learning Disabilities Team on a project basis in Bath and North East Somerset (BANES). This is an exciting opportunity for a skilled social worker to contribute to a specialized project focusing on the split of funding for adults with learning disabilities, who are currently funded by a pooled budget between the Integrated Care Board and Social Care.
Key Responsibilities:
Conducting Care Act Assessments for adults with learning disabilities to determine their eligibility for support.
Assessing Continuing Healthcare (CHC) needs and completing the relevant tools to recommend funding splits.
Collaborating with health and social care professionals to accurately determine the split of funding and ensure appropriate allocation of resources.
Maintaining thorough documentation and records in line with legal requirements and organizational policies.
Engaging with service users, families, and carers to gather relevant information and provide support throughout the assessment process.
Contributing to the development of a comprehensive plan for funding splits that supports the integrated care approach.
Requirements:
A recognized social work qualification (Degree/Diploma in Social Work).
Current registration with Social Work England.
Experience in working with adults with learning disabilities, particularly in a statutory setting.
Strong knowledge of the Care Act 2014, Continuing Healthcare (CHC) framework, and relevant legislation.
Excellent assessment, analytical, and communication skills.
Ability to work effectively in a multidisciplinary environment and manage complex cases.
