Role Overview:
As an Occupational Therapist in the Single Point of Access team, you will handle a high volume of urgent cases, particularly focusing on moving and handling needs. Your expertise will be critical in ensuring the safety and well-being of service users, addressing cases swiftly to prevent risks to individuals and the service.
Key Responsibilities:
- Urgent Case Management: Respond promptly to urgent referrals and manage cases involving complex OT needs, with a focus on moving and handling.
- Assessment and Intervention: Conduct comprehensive assessments to determine the specific needs of service users and develop tailored intervention plans.
- Risk Management: Identify and mitigate risks associated with delays in addressing OT needs, ensuring both the safety of service users and the integrity of the service.
- Collaboration: Work closely with a multidisciplinary team, including social workers, healthcare professionals, and support staff, to provide holistic care.
- Documentation: Maintain accurate and detailed records of assessments, interventions, and outcomes using council IT systems, ensuring compliance with statutory and local protocols.
- Communication: Liaise effectively with service users, families, and other professionals to provide updates and ensure coordinated care.
- Service Improvement: Contribute to the ongoing development of the Single Point of Access team, recommending process improvements to enhance service delivery.
Qualifications and Experience:
- Fully qualified Occupational Therapist, registered with the relevant professional body.
- Experience in handling urgent and complex cases, particularly related to moving and handling.
- Strong assessment and risk management skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team in a high-pressure environment.
