Location: Warrington
Pay Rate: £30 per hour
Working Hours: Monday to Friday, 9.00am - 5.00pm (excluding bank holidays)
About the Role: Warrington Borough Council is seeking a dedicated and compassionate Social Worker to join our Community Discharge to Assess (D2A) Team. In this critical role, you will be responsible for conducting comprehensive Care Act assessments for individuals discharged from hospital into temporary 24-hour care placements. Your primary focus will be to assess care needs, identify appropriate services, and coordinate the necessary care and support for each individual.
Key Responsibilities:
- Conduct thorough Care Act assessments for individuals recently discharged from the hospital, evaluating their physical, mental, and emotional needs, as well as their functional abilities.
- Develop and implement tailored care plans in collaboration with individuals, their families, and healthcare professionals.
- Provide advice, guidance, and emotional support to individuals and families, helping them understand their options and make informed decisions.
- Liaise with community resources, including healthcare providers and voluntary organisations, to ensure seamless service provision.
- Advocate for individuals' rights and entitlements, ensuring timely access to appropriate support services.
- Maintain accurate and up-to-date records of assessments, interventions, and outcomes in line with organisational and legal requirements.
- Stay informed about changes in legislation, policies, and best practices relevant to social work, care assessments, and community support services.
Requirements:
- Bachelor's or Master's degree in Social Work.
- Valid Social Work registration.
- Proven experience in conducting assessments and developing care plans, with specific experience in Discharge to Assess (D2A) being essential.
- Strong knowledge of the Care Act, Mental Capacity Act, and Best Interest processes.
- Knowledge of Continuing Health Care processes and experience in participating in DSTs is an advantage but not essential.
- Excellent communication skills, both verbal and written, with the ability to convey complex information clearly and empathetically.
- Strong organisational and time management skills, with the ability to prioritise tasks effectively.
- Strong interpersonal skills with the ability to build rapport with individuals, families, and multi-disciplinary teams.
- Proficient computer skills, including experience with electronic record-keeping systems.
- Valid driver's license and access to your own car with business insurance.
