Key Responsibilities:
- Assessment and Care Planning:
- Serve as the allocated Occupational Therapist for adults and children with physical disabilities.
- Assess, develop, review, and monitor individual care plans to support adaptations, equipment needs, and moving and handling for independent living.
- Service Commissioning:
- Be responsible for commissioning services for the provision of adaptations and equipment.
- Enable clients and their carers to achieve their full potential for independent living.
- Liaise with voluntary and statutory bodies, both locally and nationally, to ensure comprehensive service delivery.
- Duty Officer Role:
- Act as a Duty Officer, offering consultation and expert advice to internal care management teams, other health and social care professionals, and the general public.
- Provide guidance on disability-related issues, possible equipment solutions, and moving and handling techniques.
- Screen referrals into the service to ensure appropriate and timely support.
Qualifications and Skills:
- A degree in Occupational Therapy and relevant registration with the Health and Care Professions Council (HCPC).
- Experience in working with individuals with physical disabilities.
- Knowledge of adaptation, equipment provision, and moving and handling techniques.
- Excellent communication and interpersonal skills for effective collaboration with clients, carers, and other professionals.
- Strong organizational skills and the ability to manage a caseload effectively.
