Position: Qualified Adults Social Worker - South Neighbourhood Team
Location: Bolton
Pay Rate: £30 per hour
Working Hours: Monday to Friday, 9.00am - 5.00pm (excluding bank holidays)
About the Role: An exciting opportunity has arisen to join Bolton's Adult Central South Neighbourhood Team as a Qualified Adults Social Worker. In this role, you will work closely with health colleagues to support adults and their carers who may be eligible for assessment and care services under the Care Act 2014. Bolton Council is dedicated to person-centred and strength-based practice, focusing on supporting people to live well at home and creating strong, self-sufficient communities.
Key Responsibilities:
- Conduct Care Act assessments to determine eligibility for care services and develop tailored care and support plans.
- Provide ongoing support to individuals with complex social needs, addressing family dynamics, legal issues, and crisis situations.
- Complete Mental Capacity Assessments and participate in Best Interests meetings as required.
- Collaborate with a multidisciplinary team, including Social Workers, Occupational Therapists, Nurses, and Mental Health Practitioners, to deliver high-quality health and social care services.
- Participate in and lead Safeguarding Adult Enquiries, assessing and investigating risk to ensure the safety and well-being of vulnerable adults.
- Utilise resources from Social Care, Health, and other voluntary and statutory agencies to maximise individuals' independence and provide robust assessments.
- Perform all related administrative functions, attend case conferences and reviews, and represent the Council at Court when appropriate.
Requirements:
- Bachelor's or Master's degree in Social Work.
- Valid Social Work registration.
- Proven experience in conducting Care Act assessments and developing care plans.
- Strong knowledge of safeguarding practices, Mental Capacity Act, and Best Interests processes.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
- Strong organisational and time management skills, with the ability to prioritise tasks effectively.
- Proficient in using electronic record-keeping systems.
