City of Westminster are looking for an experienced and dedicated Occupational Therapist Care Manager to join their Community Independence Service (CIS) as a Reablement Coordinator. If you are passionate about supporting adults with complex health and social care needs to live independently in their own homes, this is the role for you.
The CIS is an integrated health and social care service designed to provide urgent support to individuals with disabilities or those recovering from injury, helping them build the confidence and skills needed to stay safe and independent at home. As a Reablement Coordinator, you will play a vital role in assessing, coordinating, and monitoring our customers reablement programs, ensuring they achieve their goals and minimize the need for long-term support.
Key Responsibilities:
Conduct functional assessments and develop tailored reablement programs for service users referred to CIS.
Set and review goals, coordinating support to ensure customers can safely manage daily activities with maximum independence.
Collaborate with a multidisciplinary team, including community nurses, social workers, occupational therapists, and external partners, to deliver holistic care.
Monitor and review progress weekly, adjusting care plans as needed and ensuring the effectiveness and safety of any equipment provided.
Liaise with service users, their families, and external agencies to optimize the support provided and promote the ethos of CIS.
Complete discharge assessments to identify ongoing needs and eligibility for continued support in line with the Care Act 2014.
Maintain accurate and concise records, reports, and referrals to ensure the best possible outcomes for service users.
What Were Looking For:
Knowledge and understanding of care coordination and the Trusted Assessor role within a social care/health setting.
Basic understanding of conditions leading to disability and the services available to support those affected.
Familiarity with anti-discriminatory practices and the needs of diverse communities.
Experience working within a multidisciplinary team and an understanding of Occupational Therapy/Physiotherapy in the community.
Strong communication skills, both written and verbal, with the ability to work cooperatively within a team and independently in the community.
Commitment to professional development, the ethos of promoting independence, and supporting people to remain in their homes.
Ability to prioritize tasks, manage time effectively, and produce clear and concise reports.
Qualifications:
GCSE or equivalent in Maths and English, Diploma/NVQ 3.
Ideally, the post holder will be a competent Trusted Assessor and Care Act trained.
Skills:
Exceptional communication and interpersonal skills.
Excellent problem-solving abilities.
Proficient in recording and IT skills.
Experience in completing Care Act Section 9 needs assessments
Additional Requirements:
This role will require 5 years of referencing and an enhanced DBS with checks.
