Leeds City Council is seeking experienced Qualified Social Workers to join our Learning Disabilities Care Management and Transitions teams. This role provides an opportunity to work collaboratively with individuals with learning disabilities, their carers, and various professionals, including health and provider organizations, to deliver high-quality support and care.
Key Responsibilities:
Utilize strength-based approaches to empower individuals and maximize their potential.
Partner with clients to assess needs, develop support plans, and arrange services that promote choice and control.
Develop creative support plans that deliver personalized outcomes within budgetary constraints.
Conduct safeguarding investigations, develop protection plans, and manage risk effectively.
Perform Mental Capacity Assessments and make best interest decisions where necessary.
Maintain high standards of report writing and record keeping.
Work collaboratively with colleagues across all Care Management Teams and external providers to ensure value for money through robust assessment and analysis.
Experience and Skills Required:
Minimum of 12 months post-qualification experience in a statutory setting.
Experience in complex safeguarding and risk management.
Knowledge and application of the Care Act 2014 and Mental Capacity Act 2005.
Proven ability to implement person-centred approaches and advocate for service users.
Strong problem-solving skills and creativity in developing support plans.
Excellent communication and report writing skills.
Ability to use reflective practice for decision making and planning.
Effective use of supervision for professional development.
Experience in devising, implementing, and reviewing support plans.
Possession of a full current driving licence.
Desirable Experience:
Experience working with individuals with learning disabilities.
Experience working and reporting within a court environment.
Experience with a wide range of user groups.
Qualifications:
Relevant professional qualification (e.g., BSc, BA, MA, Diploma in Social Work, CQSW, or equivalent).
Registration with Social Work England upon commencement and maintenance of registration.
