We are seeking a skilled Occupational Therapist to join the Hospital Discharge Team in Brent. In this role, you will work closely with hospital staff, patients, and their families to facilitate safe and efficient discharges from the hospital. Your primary goal will be to support patients in transitioning back to their homes or community settings, ensuring their independence and well-being.
Key Responsibilities:
Conduct assessments of patients to identify their needs for discharge planning.
Develop and implement tailored care plans to facilitate safe and timely hospital discharges.
Collaborate with multi-disciplinary teams, including doctors, nurses, and social workers, to coordinate discharge plans.
Provide advice and support to patients and their families on the use of adaptive equipment and techniques to enhance independence.
Monitor and review patient progress, adjusting care plans as necessary.
Maintain accurate and detailed patient records in line with professional standards and legal requirements.
Advise on and facilitate access to community resources and services to support patient care post-discharge.
Experience and Skills Required:
Degree in Occupational Therapy and HCPC registration.
Experience in a hospital or acute care setting, particularly with hospital discharge planning.
Strong knowledge of community care services and resources.
Excellent communication and interpersonal skills.
Ability to work effectively in a fast-paced environment and manage a varied caseload.
Problem-solving skills and the ability to develop innovative solutions for patient care.
Good organisational and time management skills.
