We are seeking an experienced and qualified Social Worker (QSW) to join our Adult Community Team (ACT) in Newcastle. The successful candidate will be responsible for providing comprehensive support to adults in the community, ensuring their needs are met and that they receive the best care and safeguarding.
Key Responsibilities:
Conduct assessments and provide care in line with the Care Act.
Implement and ensure compliance with the Mental Capacity Act (MCA).
Address safeguarding concerns, working to protect vulnerable adults.
Apply the 3 Conversation Model where possible.
Collaborate in Continuing Healthcare (CHC) assessments to ensure appropriate care is provided.
Key Requirements:
Qualified Social Worker with a valid HCPC registration.
Experience in adult social care, including working with the Care Act, MCA, and safeguarding.
Familiarity with the 3 Conversation Model is desirable.
Experience with Continuing Healthcare (CHC) is a plus.
Strong communication and teamwork skills.
