Leeds City Council is seeking a skilled Advanced Practitioner (AP) to join the Hospital Discharge Neighbourhood Team South on a temporary basis, providing essential cover while we recruit permanent staff. This role focuses on facilitating safe and effective discharges from hospital and ensuring individuals receive the necessary care and support in the community.
Key Responsibilities:
Work as part of the Hospital Discharge Team, managing complex cases and ensuring the smooth transition of individuals from hospital to their homes or community settings.
Conduct assessments of service users' needs, care plans, and risk management, ensuring that all processes meet statutory requirements.
Collaborate closely with health professionals, social workers, and other multidisciplinary teams to facilitate safe hospital discharges.
Coordinate and deliver high-quality care and support plans, ensuring continuity of care and a person-centred approach.
Manage a caseload of individuals requiring timely interventions, prioritising tasks effectively to ensure that individuals are discharged safely.
Travel within the Leeds area to complete home visits, community assessments, and post-discharge follow-up.
Provide supervision and guidance to other members of the team when required.
Uphold the values of Leeds City Council, ensuring compliance with relevant legislation, policies, and procedures.
Requirements:
Qualified Social Worker with registration to Social Work England.
Previous experience in hospital discharge or related adult social care settings is essential.
Strong understanding of discharge processes, care planning, and working with health and social care professionals.
Excellent communication, organisational, and decision-making skills.
Ability to manage complex cases, work under pressure, and adhere to deadlines.
A drivers licence is desirable but not essential travel around Leeds will be required as part of the role.
