We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Adult Frailty & Long Term Conditions (AFLTC) team in Lincolnshire. This role involves working closely with adults to conduct strength-based assessments, ensuring that individuals receive the care and support they need to maintain their independence.
Key Responsibilities:
Conduct strength-based assessments for adults with frailty and long-term conditions.
Develop and implement care plans that are person-centred and focused on maintaining independence.
Apply a thorough understanding of the Community Care Act and Mental Capacity Act, as well as other relevant social care legislation.
Work collaboratively with service users, their families, and other professionals to deliver high-quality care.
Recognize and respond to safeguarding concerns in a timely and appropriate manner.
Maintain accurate and up-to-date records in line with legal and organizational requirements.
About You:
You must be a Qualified Social Worker with valid registration.
Strong knowledge of the Community Care Act and Mental Capacity Act is essential.
Experience in working with adults, particularly those with frailty and long-term conditions, is highly desirable.
You should be committed to working in a person-centred way, empowering service users to make informed decisions about their care.
Excellent communication and interpersonal skills are required to effectively collaborate with a range of stakeholders.
