We are seeking an experienced Qualified Social Worker (QSW) to join our Adult Services team in Bolton. The ideal candidate will have a strong background in completing Care Act Assessments and developing support plans for adults with a wide range of needs. You will play a key role in providing high-quality, person-centered support, ensuring service users make informed decisions about their care while balancing risk and safeguarding responsibilities.
Key Responsibilities:
Care Act Assessments: Complete thorough assessments in line with the Care Act 2014, addressing a variety of adult needs.
Support Plans: Develop, implement, and review personalized support plans, facilitating appropriate care arrangements for service users.
Carer's Needs: Understand the rights of Carers and conduct Carer's assessments to provide tailored support.
Integrated Working: Collaborate with multi-disciplinary teams to deliver seamless support and care to service users.
Safeguarding: Identify, assess, and manage safeguarding risks, ensuring service user safety while respecting their autonomy.
Service User Advocacy: Advocate for and assist service users in representing their needs, views, and circumstances.
Reporting & Documentation: Prepare and present reports for decision-making forums, ensuring service users are involved and informed in the process.
Duty Function: Contribute to the duty function of the team as required.
Essential Skills and Experience:
Social Work Expertise: Experience in Adult Social Work and Care Act Assessments, with a focus on safeguarding and risk management.
Communication Skills: Ability to engage and maintain professional relationships with service users, advocating on their behalf where necessary.
Teamwork: Proven experience working effectively within a multi-disciplinary team and across integrated systems.
Problem-Solving: Strong skills in assessing situations, identifying needs and urgency, and implementing appropriate interventions.
Report Writing: Competent in preparing detailed and accurate reports for decision-making.
Knowledge: Familiarity with the Liquid Logic system would be beneficial but is not essential.
Requirements:
Qualified Social Worker registered with Social Work England.
Previous experience in a social work setting, particularly with Care Act Assessments.
Knowledge of safeguarding and risk management in adult care.
3 years of professional references required, and gaps in employment history will be reviewed.
