Nottinghamshire County Council is seeking an experienced Qualified Social Worker (QSW) to join our Continuing Healthcare (CHC) team. This is a fast-paced and demanding role that requires a skilled professional with a thorough understanding of joint funded support and Continuing Healthcare (CHC) processes.
Key Responsibilities:
Provide expert support in joint funded care, ensuring that services are delivered in line with CHC requirements.
Participate in Decision Support Tool (DST) meetings, ensuring thorough assessments and clear decision-making processes.
Deliver one-to-one support for individuals and families navigating the CHC process, providing expert advice and guidance.
Work collaboratively with health colleagues, promoting a strengths-based approach to achieve positive outcomes for individuals.
Negotiate effectively with healthcare professionals to secure the right support for service users.
Manage a fast-paced workload, ensuring that deadlines are met and high-quality services are delivered.
Essential Criteria:
Qualified Social Worker with HCPC registration.
Proven experience in joint funded support, Continuing Healthcare, and Decision Support Tool processes.
Strong negotiation skills and ability to work effectively with health professionals.
Experience working in fast-paced environments, managing complex caseloads with confidence.
A commitment to strengths-based, person-centred care.
