The Nottingham Community Prevention Team (Adult Social Care) is seeking a dedicated and experienced Qualified Social Worker to join their team. This team plays a key role in supporting citizens with multiple long-term conditions, including neurological disabilities, by offering ongoing case management to reduce the risk of avoidable hospital admissions or long-term care placements.
Key Responsibilities:
Work with citizens with complex needs and long-term conditions, providing ongoing case management.
Collaborate with a Primary Care Network (PCN) and other multi-disciplinary teams (MDTs) responsible for citizens within the locality.
Develop and implement holistic support plans tailored to the unique needs of each citizen.
Promote preventative care, reducing the likelihood of hospital admissions or long-term care by providing effective, person-centred support.
Liaise with health and social care professionals, families, and carers to coordinate comprehensive care packages.
Requirements:
Must be a Qualified Social Worker (QSW) registered with Social Work England.
Proven experience working in Adult Social Care or in a similar setting, particularly with citizens with complex health needs.
Strong skills in case management, risk assessment, and support planning.
Ability to work effectively within a multi-disciplinary team and across networks.
