Job Title: Advanced Practitioner - Discharge to Assess

Job Overview:
Nottinghamshire County Council is seeking a skilled Advanced Practitioner to join the Discharge to Assess team. In this pivotal role, you will facilitate the safe and timely discharge of patients from hospital settings, providing person-centered care and enabling clients to transition back to their homes or community environments with adequate support. This role involves multi-agency collaboration, focusing on maximizing independence and streamlining discharge processes.
Key Responsibilities:
Conduct comprehensive assessments of patient needs upon discharge, ensuring appropriate support and care plans are implemented.
Coordinate and manage complex discharge cases in collaboration with multi-disciplinary teams to facilitate timely, safe discharges.
Work within the Discharge to Assess framework to ensure seamless transitions from hospital to home or alternative care settings.
Implement strengths-based and person-centered approaches, empowering clients to maintain independence in their communities.
Act as a point of escalation for complex cases and provide support and guidance to other team members.
Ensure all documentation and care plans comply with Nottinghamshire County Council policies and statutory requirements.
Liaise with other agencies, care providers, and families to provide comprehensive, ongoing support post-discharge.
