About the Role
We are seeking a Qualified Social Worker to join the Adult Transition and Childrens Leaving Care Team at Achieving for Children, based in Maidenhead. This position offers an opportunity to make a meaningful impact in the lives of young people transitioning into adulthood.
Key Responsibilities
Transition Planning: Work with young people preparing for adulthood to ensure smooth transitions to adult services, focusing on education, employment, housing, and care needs.
Care Leaver Support: Provide targeted support to care leavers, promoting independence and ensuring their voices are central to planning and decision-making.
Collaboration: Work in partnership with families, carers, and multi-agency professionals to deliver high-quality support tailored to the young persons needs.
Statutory Compliance: Ensure that all work adheres to statutory guidelines and organizational policies, delivering exceptional standards of practice.
Hybrid Working: Attend the office 2-3 days per week, while balancing in-person visits and remote work.
Key Requirements
Qualifications:
Degree in Social Work or equivalent.
Active registration with Social Work England.
Experience:
Proven experience in adult transitions or working with care leavers.
Familiarity with relevant legislation, including the Care Act and Children (Leaving Care) Act.
Demonstrated ability to engage effectively with young people and families.
Skills:
Strong organizational and time management skills to handle a varied caseload.
Excellent communication and relationship-building skills with young people and professionals.
Proficiency in assessment, planning, and delivering care plans.
Transport:
Must be able to drive and have access to a car for work purposes.
Working Arrangements
Location: Based at Maidenhead Town Hall.
Hybrid Schedule: 2-3 office days per week, with flexibility to conduct fieldwork and work remotely.
