Occupational Therapist - Intermediate Care Service Reviews Team
Location: Walsall
Rate: £29.99 per hour
Contract: Temporary

We are seeking a skilled Occupational Therapist to join Walsalls Intermediate Care Service Reviews Team, focusing on reviewing care packages for individuals following hospital discharge.

Role Overview
As part of a multidisciplinary team (MDT), you will:

Conduct initial and subsequent reviews of care packages at home post-discharge.
Use a strengths-based approach to complete:
OT assessments/reviews.
Moving and handling assessments.
Single-handed care assessments.
Promote independence by assessing for and providing equipment, assistive technology, and community asset signposting.
Develop revised support plans if care needs change, and complete risk assessments as necessary.
Key Requirements
HCPC-registered Occupational Therapist with relevant experience.
Strong understanding of promoting independence through strengths-based practice.
Familiarity with equipment provision and community resources.
Excellent communication and organizational skills.
Ability to work effectively within an MDT environment.
