About the Role:
Kirklees Council is offering an exciting opportunity for experienced Qualified Social Workers (QSW) to join a transformational project. The focus is on undertaking strength-based reviews for adults aged 18 and over within the Kirklees community. This role is pivotal in promoting individuals' autonomy, strengths, and independence, enabling them to live fulfilling lives within their communities.
As part of a dedicated and dynamic team, you will manage your own caseload with the support and oversight of management while collaborating with partners across health and social care services.
Key Responsibilities:
Conduct strength-based reviews to assess and enhance the independence of individuals in the community.
Work collaboratively with adults, their families, and carers to identify and utilize their strengths, networks, and resources.
Develop person-centered plans that focus on promoting well-being and achieving positive outcomes.
Liaise effectively with allied health and social care colleagues, as well as other stakeholders, to deliver integrated and coordinated support.
Manage and prioritize your caseload with support and guidance from management.
Maintain accurate and up-to-date case records, ensuring compliance with relevant policies and legislation.
What We're Looking For:
A qualified social worker, registered with Social Work England.
Strong knowledge and experience in strength-based approaches and adult social care.
Comprehensive understanding of relevant legislation, including the Care Act 2014.
Excellent interpersonal, assessment, and organizational skills.
A proactive and collaborative mindset, with the ability to work effectively with a range of professionals and stakeholders.
Commitment to improving outcomes for adults and promoting their independence.
