Team/Service Overview:
Join our dynamic Learning Disability Team and make a real difference in the lives of adults over the age of 18 with learning disabilities. The team plays a key role in supporting transitions for young people into Adult Social Care, working closely with Children's Services, the Special Educational Needs (SEN) Team, and the Integrated Health Care Board.
We are committed to person-centred support underpinned by the Care Act (2014) and Mental Capacity Act (2005), and we work closely with health colleagues in a co-located setting. You will contribute to assessments, develop support plans, and review care packages while promoting independence and safeguarding the most vulnerable.
Key Responsibilities:
Conduct assessments and develop person-centred support plans under the Care Act (2014).
Apply the principles of the Mental Capacity Act (2005), including capacity assessments and Court of Protection work.
Support people detained under the Mental Health Act (1983) and complete Decision Support Tool meetings for Continuing Health Care (CHC) eligibility.
Identify and address safeguarding concerns, including self-neglect, abuse, and neglect.
Work collaboratively with health colleagues, families, and external agencies to achieve the best outcomes.
Essential Skills and Experience:
Proven experience in a Learning Disability Social Care Team.
Knowledge and practical application of the Care Act, Mental Capacity Act, and Community Deprivation of Liberty Safeguards (DoLS).
Excellent written and verbal communication skills.
Strong ability to manage caseloads effectively with support through regular supervision.
