Job Title: Senior Social Worker - Hospital Team
Pay Rate: Upto £36 per hour
Location: Lewisham Hospital, Lewisham SE13
Role Overview
Lewisham Council is seeking an experienced Qualified Social Worker to join their dynamic Hospital Team. The role involves supporting hospital discharges, undertaking assessments, and providing high-quality care planning for adults in need. The focus is on promoting independence, safeguarding individuals, and ensuring seamless transitions from hospital to community settings.
Key Responsibilities
Hospital Discharge Planning:
Work closely with healthcare professionals to coordinate timely and safe discharges from the hospital.
Identify support needs and arrange appropriate care packages.
Care Act Assessments:
Undertake strength-based assessments in line with the Care Act 2014 to evaluate the needs of individuals and their carers.
Safeguarding:
Carry out safeguarding screenings and enquiries to ensure individuals are protected from harm and abuse.
Care Planning:
Develop person-centered care plans that promote independence and well-being.
Review and adjust care plans as necessary to meet changing needs.
Multi-Agency Collaboration:
Liaise effectively with healthcare providers, community services, and families to deliver holistic support.
Candidate Requirements
Qualifications:
Social Work Degree or equivalent.
Registered with Social Work England.
Experience:
Proven experience in adult social care, hospital discharge, and safeguarding.
Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and relevant health and social care legislation.
Skills:
Excellent assessment, care planning, and report-writing skills.
Ability to manage a busy caseload and meet statutory deadlines.
Effective communication and relationship-building skills.
Personal Attributes:
Compassionate, proactive, and able to work collaboratively within a multi-disciplinary team.
