North Lincoln Council is seeking a dedicated Qualified Social Worker (QSW) to join our Learning Disabilities Social Work Team. This role involves carrying out social work duties under the Care Act, supporting individuals with learning disabilities to achieve the best possible outcomes.
Key Responsibilities:
Manage a caseload of individuals with learning disabilities, ensuring their needs are met in accordance with the Care Act.
Conduct assessments, care planning, and reviews to promote independence and well-being.
Work collaboratively with families, carers, and multi-disciplinary teams to provide person-centred support.
Ensure effective safeguarding measures are in place.
Maintain accurate and up-to-date records in line with council policies and procedures.
Requirements:
Qualified Social Worker (QSW) with relevant registration.
Proven experience in learning disabilities social work.
Strong knowledge of the Care Act and safeguarding practices.
Ability to work effectively within a multi-disciplinary team.
Excellent communication and organisational skills.
