Role Overview: We are seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Living Well (North) team. This role involves working with adults who require support to live independently, providing assessments, and implementing care plans to enhance their well-being. You will be part of a dynamic, person-centred team that values innovation and collaboration.
Key Responsibilities:
Conduct comprehensive assessments of individuals needs and develop tailored support plans.
Work closely with service users, families, carers, and multi-disciplinary teams to promote independence and well-being.
Ensure that safeguarding policies and procedures are followed to protect vulnerable adults.
Advocate for service users and provide professional advice and support.
Maintain accurate records and reports in line with statutory requirements.
Participate in team meetings, training, and professional development activities.
Requirements:
Degree in Social Work and registration with Social Work England.
Experience working in adult social care, preferably in a community setting.
Strong knowledge of relevant legislation, including the Care Act 2014 and the Mental Capacity Act 2005.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
A commitment to promoting independence and person-centred care.
Full UK driving licence and access to a vehicle (desirable).
