Qualified Social Worker - Adult Social Care Team
Location: Wokingham Borough Council
Rate: £35.89 per hour
Contract: Locum
Wokingham Borough Council is currently seeking an experienced Qualified Social Worker to join their Adult Social Care Team (18+).
The role involves working directly with adults who have a learning disability, physical disability, long-term conditions, or are transitioning into adulthood. The team also supports young carers and older people, aiming to empower individuals to live as independently and safely as possible in the community.
Key Responsibilities:
Undertake strengths-based assessments, reviews, and support planning with individuals and their carers.
Support people to identify their own needs and outcomes and co-produce plans to meet these.
Work collaboratively with multi-agency professionals to promote well-being and independence.
Ensure all work aligns with relevant legislation and guidance, including the Care Act 2014, Mental Capacity Act, and safeguarding frameworks.
Requirements:
Qualified Social Worker with Social Work England registration.
Minimum of two years post-qualifying experience in Adult Social Care.
Strong understanding of relevant legislation, care planning, and person-centred practice.
