Rochdale Borough Council is looking for skilled agency professionals to join our Mental Health Review Team and Community Mental Health Team (CMHT West).
This role is vital to the delivery of high-quality, statutory adult mental health services. You will be part of a multi-disciplinary team, co-located with health colleagues, ensuring joined-up and person-centred support.
Key Responsibilities:
Undertake statutory Care Act assessments, scheduled and unscheduled reviews, and update care plans.
Complete Mental Capacity Assessments and contribute to best interest decision-making.
Carry out Safeguarding investigations in line with s42 of the Care Act.
Commission appropriate services to meet assessed needs.
Support hospital discharges and transitional planning for individuals with severe and enduring mental ill-health.
Work with individuals both in-borough and placed out-of-borough.
Essential Requirements:
Qualified Social Worker with evidence of relevant qualifications
Social Work England registration
Minimum 5 years post-qualifying experience
Experience within a statutory adult social care mental health setting
Knowledge of:
Care Act 2014
Mental Health Act
Mental Capacity Act
Human Rights Act, Equalities Act, GDPR, CHC frameworks
DoLS and safeguarding legislation
Experience of:
Completing Care Act assessments and reviews
Writing and commissioning care plans
Conducting MCA assessments and safeguarding enquiries
Full, clean driving license
2 x satisfactory references
No unexplained gaps in employment history
Desirable:
Experience writing Social Circumstance Reports for Tribunals and Manager's Hearings
Understanding of risk management in mental health settings
Knowledge of mental health care pathways
Working Conditions:
37 hours per week, Monday to Friday
Primarily office-based due to the client group's needs
Some home working available with management agreement
